Sleep Apnoea Medical Card for Wallets/Purses
The card is the same size and material as a credit card, and has the added convenience of strips to enable writing on it with a normal pen.
The picture show a front view and a rear view, and the wording on the front of the card reads:-
"I have the medical condition, Sleep Apnoea, and need to use a CPAP Machine giving me 'continuous positive air pressure' to keep my airways open when asleep or unconscious."
There is a place to write your Name, Address, Tel No + Date of Birth
On the back of the card the information to fill in is:-
Emergency Contact, GP Information, Hospital/Sleep Clinic Information and CPAP Pressure.
