Sleep Apnoea Forum Bringing Help + Support to the Patient

I have been on CPAP therapy for 3 years, recently I am feeling very tired in the daytime.  This morning my machine showed 8 hours 28 minutes and no leaks.  Use a ResMed automatic machine and a full face mask.  An odd time if I turn in my sleep my mask leaks this happens occasionally.  I am age 70 if that makes any difference. Any advice would be really appreciated.

Are your AHI results on the CPAP 5 or below? If so, check your hose hasn't developed any little holes. Occasional mask leaks shouldn't cause this.

If all is well with your CPAP therapy and you're getting 7-9 hours sleep every night then it would be worth getting blood tests for vitamins D, B12, thyroid and diabetes which all cause tiredness and are linked with sleep apnoea.

Hi Kath thanks for your response, I have been given vitamin D tablets for a year and also a few months ago was on Iron and folic acid tablets.  I had a Overactive Thyroid in my 30s and have no thyroid so I am on Thyrioxine dailly which is checked regularly.  I can't check my AHI results as my machine is locked by the clinic who supplies it.  How long on average should  FULL Face mask last.

Just a thought Mags, but if the tiredness has come on you since you came off the iron/folic tablets, maybe that could be the cause?  Also a lot of people do need large doses of Vitamin D3 and I wonder if that could be affecting things now that winter's set in.  These are only my personal thoughts by the way.

Thanks Kath, will see the GP in the New Year I wondered myself if it was Iron.  I find that the GP doesn't seem to know very much about OSA, and never do any checks on it.

Hi, i too suffer from daytime tiredness, so much so that if I sit at any time of day for more than aboiut 20 minutes I am overcome by waves of sleepiness. I suddenly fall into a deep sleep so quickly that I drop mugs of tea or anything else.

According to my stats I am getting about 7 hours sleep in every 24 hours consistently over several years. My problem is that my sleep is broken up into 90 minute blocks. I could set my watch by the length of my sleep. Once I wake I am then awake for about 90 minutes - at which point I am exhausted and just have to sleep again..

I have tried to overcome this and enforce a 'normal' routine but after a few days I am just unable to function. I usually wake needing the bathroom. I often wake with an awareness that my machine is pulsing quick sharp bursts as if it is trying to wake me up deliberately.. This has been going on for several years and I don't know where to turn. My AHI is avg 1.9 to 2.3.

I use Sleepyhead software to monitor my stats but tbh I don't understand it at all.

I get reports periodically from my hospital saying that my sleep apnea is well controlled and I am afraid to tell them any different. I live in a very rural area and depend entirely on having my own car to go shopping etc. I have limited mobility so I couldn't use buses or trains to get about.

Any advice would be most welcome.

Bob.

Sorry Bob, but I missed your post.  If you're having broken sleep this will cause daytime tiredness.  Roughly every 90 mins we go into REM sleep and I'm wondering if you're being woken up by the pressure rising when apnoeas start (if you're on auto pressures) or if an apnoea itself is waking you if you're not getting enough pressure (if you're on fixed pressures).

Hi Kath,

I think, looking at the graphs that I have a very low flow rate and then an OA event and then the machine sends out one or more pulses. This is almost invariably when I wake. So it could be the OA event waking me (or waking causing the OA), or the machine pulsing which wakes me?

I'm confused - it looks a bit like a 'chicken - egg' situation and beyond my understanding.

I do drop into cycles of very shallow breathing and I do have 'cheyne-stokes' periodic breathing events?

any suggestions?

Thanks. Bob.

Hi Bob (delayed reply as the forum wouldn't post last time I tried).  Your average pressure is certainly very low.  I'm assuming it's auto pressures (APAP)?  Your AHI is good too.  It would be better to upload a graph of a daily report rather than an exel file though.  Something like this one:-

A tale of two nasal dilators
