Sleep Apnoea Forum Bringing Help + Support to the Patient

On Resmed's RealSleep uk site, I've just come across a sample CPAP report for an S9.  If I've understood the report correctly it looks as if the minimum pressure was set at  4 and maximum at 20, and if I remember correctly the report covered 38 days of machine use.  Are Resmed suggesting it is OK to keep the pressure settings wide open?

Yes, they are! Some people actually do it - but they seem to average very low pressures <10 or 11. For higher pressure users of APAP - more likely to have 'strong' events - it seems best to raise the Pmin a lot and lower the Pmax a bit.

 

Hmmm. That's a generic APAP answer - mayhap the S9 has a magical algorithm?

A magical algorithm - now that would be something. Still the machines are getting cleverer, much better than what I remember from 15+ years ago.

 

I have been using an S8 Spirit11 for the last 6 months.  In the beginning I had it set wide open 4 to 20. (On straight CPAP I would be about 13.4).  Over several months use the AHI came down to about 5, but not below, mostly hypopneas.  I don't have the software to check nightly flow readings at home, but I had several check-ins with my sleep clinic, which showed I did a lot of breath holding. I suspect that I increase the breath holding if I have too much air coming at me, but I don't know this for certain.

 

 Does breath holding morph into central apnoeas for some individuals?

 

Over the last few months I have raised the Pmin in stages and brought the Pmax down to 18. Pmin is now at 12.4. Each time I moved the Pmin, my AHI would rise a bit before dropping after a few days. The previous setting I was using was the first that took my AHI below 5. The 12.4 setting is still giving a higher reading than 5, but dropping daily.  So, as you see, I am a work in progress!

Aren't we all a work in progress!! I think PAPing is much like a woman's work .........
Lisa, Breath holding or swallowing do not morph into centrals. These natural events may however be counted as breathing events by the Resmed algorithm. I have been told NOT to put so much emphasis on the HI in your particular machine for this reason.
