I started on cpap last april with an ahi of 34+. By Christmas I was down to 19. New year onwards ahi's have gone back up to 25. I am not due for clinic until june. The last time I saw the sleep nurse she said to take no notice of the numbers, it's how I feel that matters!!! I don't feel actually tired but I feel exhausted all the time, but not sleepy. I don't want to phone her as she gets stroppy with me as I phoned her quite a lot when I first started. Also my machine seems to turn itself off and wake me in the night. I don't know what to do as I know I will start crying when she gets stroppy. She used to be really nice but I think she thinks I nag for no reason. Believe me I don't.

Carolyn, if your machine is switching itself off in the night then there's a problem, so you will need to ring the clinic. If you're feeling too nervous then how about getting your husband or someone else to ring for you. Perhaps they could explain why you're too worried about riinging them at the same time.
Regarding your AHI's there could be a number of reasons why they've gone up, but before you get worried about them you need to get your machine sorted, as if it's faulty it could also be giving false readings.

Your machine turning off is an immediate issue that has to be taken care of. I truly believe that your AHI issue is sourced from the opiates you take. I can't tell you what to do as that could put my own license in danger. In a clinical setting the effects of opiates on the respiratory drive is a very distinctive pattern within the PSG data. If I were to see this type of pattern I would immediately initiate oxygen treatment and a slow PAP titration. If you have not yet received an in-lab titration I suggest that you push for one.
I phoned the sleep clinic today. The secretary answered and I explained my problems. She said she would phone me, probably, monday, or sometime next week to tell me if she could get me an appointment sooner than my planned June appointment. I'll give them until tuesday lunch time and then I will phone again. This is something that I really have to dig my heels in for. Last time I saw the nurse, she got stroppy and said she would refer me back to specialist doctor. I said I didn't want that. Now I wish I had. I'll keep you informed.
